Please find below a number of pro forma sheets that are designed to assist you with planning and preparation for your supply assignments. If there is something that you feel would be useful that is not in this section, please contact your local branch.
Lesson Plan Proforma
Download a blank lesson plan sheet that you can use to record the activities you will use in your lessons.
Communication Sheet
Download a communication sheet to leave for return of permanent teacher on completion of supply assignment. This sheet will contain details of work completed, pupils' behaviour and any other relevant information. Completing these forms often results in extremely positive feedback from the schools you work at and regularly results in you being requested back.
Timesheet
Download a timesheet for the end of your supply booking. We do have an online timesheet approval service, which may or may not be used by the schools you work in. Please bring timesheets along in case the schools you attend are not using our online service. Handy Hints for Supply Teachers
